The information below is to assist incoming first-years in their transition to college athletics. Below, please find key information regarding forms that need to be completed for you to compete this academic year. Sports Medicine
- Checklist for Pre-Participation Exams
- New Student-Athlete Paperwork Instructions
  - YouTube Tutorial
  - Due dates: August 1st for Football. August 8th for other sports
- Sickle Cell
  - New student-athletes/transfers are required to submit a copy of their sickle cell testing done at birth, or a recent test.
  - A form can be submitted here.
  - Newborn test results can be requested from pediatrician, hospital or State Department of Health.
- ADD/ADHD Exception Form
  - Due date: August 5th
- Impact Testing
  - Due date: August 5th

  - Please read carefully with your guardian to ensure you understand insurance requirements

Annual Attestation and Title IX Prevention and Education
- All student-athletes must complete the NCAA annual disclosure form prior to participating in any team meeting, practice or competition.
